Inubushi Keiko (born 30 April 1991) is a notorious criminal who was the perpetrator of the mass murder incident at Awazumi Hospital. The investigation into the atrocity showed that Keiko suffered from Dissociative Identity Disorder (DID) and she was absolved of murder. She represents the Shadow.
Due to the nature of the incident and her disorder, Keiko was confined indefinitely to SPHIA where she resides for the entirety of the story under the name of Suzukage Hotori. As would be expected, her personality and actions are erratic at best – to Kokoro, she is completely mute; to Satoru, she is a capricious figure, alternately flirtatious and haughty.
Inubushi Keiko is ultimately shown to be subject to the same personality exchange phenomenon as Satoru and Kokoro. The 'silent' Keiko that Kokoro meets is actually the personality of the real Suzukage Hotori. The Keiko that Satoru knows is the true Keiko; while a genuine sufferer of DID, she retains her murderous potential up to the novel's final scene. She denies being Keiko and goes by the name "Hotori" to deceive those around her.

- Alice: A young girl that searches for her brother and is a victim of abuse, heavily implied to be Inubushi's alter. [1]

- Childhood: An innocent, but extremely quick to cry six-year-old boy. [3]
- Darkness: A melancholy young man that withdraws from social interaction and has a habit of self-harm. [3]
- Empty: A complete lack of will. Can be seen in-game when Inubushi freaks out and stares off into space. [2]
- Frau: A woman in her late twenties who is extremely lewd and even seduced the doctor examining her. [3]
- Gaily: A bubbly happy young girl. [2]
- Hummingbird: A 19 year old girl with a shy and calm character. She had a fondness for listening to music. Kokoro believes this is the "Hotori" personality. [3]
- Intelligence: A smart older man. [2]
- Jack: A murderous monster whose name likely refers to Jack the Ripper. A 17-year-old woman that possesses strong destructive urges and sadistic hobbies. While held in an examination room, Jack used a pair of scissors to kill the doctor examining her. [3]
- Keiko: The main personality that controls the body. [3]
